Case Manager-works with eligible children and their guardians, facilitates the Child and Family Team to develop an Individualized Support Plan which identifies strengths and needs of the children and their families, arranges for and manages and monitors appropriate service delivery, and helps draw participation from natural supports. The position requires frequent travel to the families’ home and their community, meeting the child and family where they are most comfortable. Minimum of a Bachelor’s Degree in a specialized field of study such as, Special Education, Social Work, Psychology, Counseling, Nursing, Mental Health and Human Services, Behavioral Science or a closely related field. A closely related field may be considered with one year of professional experience working with children with mental health, mental retardation, or medical needs.

Hiring locations – Machias (half-time position)
Family Support Partner – will provide support for caregivers, youth and other family members of youth who are clients receiving Case Management services and Behavioral Health Home Services. The FSP will provide direct support, advocacy, coaching and mentorship with families to facilitate communication and their ability to fully participate in the Wraparound Individual Support Plan. High school diploma or equivalent. Experience parenting a youth involved in multiple systems of care.
