Children’s Case Manager $2,000 sign on bonus!

Utilizing the Wraparound Planning Process and the Sanctuary Model commitments and tools, the Case Manager/Care Coordinator works with eligible children and their guardians to assist them in identifying their strengths and needs; facilitates the Child and Family Team to develop an Individualized Support Plan which addresses the strengths and needs of the children and their families, arranges for and manages and monitors appropriate services delivery, and helps draw participation from natural supports. There is a large amount of documentation that must be completed on time to ensure services are continues and billing occurs. The position is primarily based in the home and community, meeting the child and family where they are most comfortable
Family Support Partner (FSP) – Ellsworth (1)
We are seeking a person who is a parent or guardian who has experience accessing services and supports for their child with special needs. The FSP works in partnership with the Care Coordinator to provide direct, non-clinical support to parents to help ensure an individualized approach which maximizes family voice and choice. High School Graduate/GED or equivalent.
Anticipated Future Openings for Adult Case Manager for People with Developmental and Intellectual Disabilities – Bangor (1) and Machias (1) Full-time positions, $2,000.00 sign on bonus!
Utilizing the Wraparound Planning Process and Sanctuary Model commitments and tools, the Adult Case Manager works with eligible adults and/or their guardians to assist them in identifying their strengths and needs; facilitates teams to develop a Person Centered Plan which addresses the strengths and needs of the person, arranges for, manages, and monitors appropriate service delivery, and helps draw participation from natural supports. There is a large amount of documentation that must be completed on time to ensure services are continued and billing occurs. The position is primarily based in the home and community, meeting the person where they are most comfortable.
Clinical Supervisor – Bangor (1) $2,500.00 sign on bonus!
The Clinical Supervisor is the licensed clinical staff person who is responsible for the direct clinical supervisor of Care Coordinators. The Clinical Supervisor may also provide case management services to clients of the program as necessary due to staff vacations, leave of absences, etc. The person in this position will adopt, implement, and promote Wraparound values including the agency philosophy, as well as the Sanctuary Model commitments and tools to guide everyday practice in the course of employment with the agency.
