Children’s Case Manager
Utilizing the Wraparound Planning Process and the Sanctuary Model commitments and tools, the Case Manager/Care Coordinator works with eligible children and their guardians to assist them in identifying their strengths and needs; facilitates the Child and Family Team to develop an Individualized Support Plan which addresses the strengths and needs of the children and their families, arranges for and manages and monitors appropriate services delivery, and helps draw participation from natural supports. There is a large amount of documentation that must be completed on time to ensure services are continues and billing occurs. The position is primarily based in the home and community, meeting the child and family where they are most comfortable
Ellsworth (1 position) Machias (1 position)
Adult Case Manager
Utilizing the Wraparound Planning Process and the Sanctuary Model commitments and tools, the Case Manager works with eligible adults with developmental disabilities and/or their guardians to assist them in identifying their strengths and needs; facilitates teams to develop a Person Centered Plan which address the strengths and needs of the person, arranges for, manages and monitors appropriate service delivery, and helps draw participation from natural supports. There is a large amount of documentation that must be completed on time to ensure services are continued and billing occurs. The position is primarily based in the home and community, meeting the person where they are most comfortable.

The Clinical Supervisor is the Licensed Clinical staff person who is responsible for the direct clinical supervision of Case Managers. The Clinical Supervisor may also provide case management services to clients of the program as necessitated, due to staff vacations or leaves of absence.
