Children’s Care Coordinator – $3,000 sign on bonus!

BA/BS in the social science field, or a closely related field with one year of experience working with children who have special needs.

We are actively hiring children’s Care Coordinators/Case Managers in the Ellsworth office. We are taking resumes for future opportunities in Bangor, Northern Penobscot County area, Presque Isle, Machias, Ellsworth, and Waterville and surrounding areas.

Utilizing the Wraparound Planning Process and the Sanctuary Model commitments and tools, the Case Manager/Care Coordinator works with eligible children and their guardians to assist them in identifying their strengths and needs; facilitates the Child and Family Team to develop an Individualized Support Plan which addresses the strengths and needs of the children and their families, arranges for and manages and monitors appropriate services delivery, and helps draw participation from natural supports.  The position is primarily based in the home and community, meeting the child and family where they are most comfortable

Family Support Workers 

We will be hiring Family Support Partners (FSP) ! Do you have lived experience as a parent of a child with unique needs? Have you navigated the children’s system for services, special education, or Department of Corrections? Or have experience working with the crisis system ? Those can be lonely waters to paddle. Most importantly, do you want to help other parents navigate those waters?
